Te Awakairangi Health Network
PHOHutt Valley District Health Board, Te Awakairangi Health Network and Cosine PHO work as an alliance and have collaboratively developed and funded the Hutt Valley Health Care Home programme in the district. In addition to the National Health Care Home model of care requirements, the Hutt Valley programme also focuses strongly on equity, consumer co-design and community integration. The Hutt Valley District currently has 15 active Health Care Homes, with approximately 80% of the local enrolled population with one of these general practices.

Community ProviderStroke Aotearoa New Zealand is the national, not-for-profit charity leading Aotearoa's fight against stroke. Established in 1979 from a partnership of volunteers and rehab professionals, it has since grown into the primary voice for stroke prevention, awareness, and support across the country.
