Health Hawke's Bay
PHOHealth Hawke's Bay is the region's Primary Health Organisation (PHO), covering a population of over 178,000. We aim to improve health outcomes and life expectancy across our beautiful bay, as well as improving equity for our Māori, Pacific and high needs families. We are a publicly funded organisation with a Board and CEO who are passionate about health and Hawke's Bay, and who are supported by a dedicated team of professionals in a wide range of roles to ensure the stainability of our health services. Health Hawke's Bay believes making it easier for people to access health care is fundamental to improving the health of our community. Health is a community and personal responsibility and Health Hawke's Bay is committed to helping you to take care of yourself, your families and whānau.
Mahitahi Hauora
PHONeighbourhood Healthcare Homes is a collaborative programme funded by Northland DHB and Te Kaupapa O Mahitahi Hauora. The regional programme is led by the improvement team at Mahitahi Hauora. The programme has evolved over the past four years with the current model aligning with the HCH Pae Ora MoC and Whakamaua the Māori Health Action Plan. The model is offered in 15 general practices with a combined population of 109,478 with 34% of our population being Māori. This equates to over 60% of the total enrolled population in Northland. The model of care in Northland is strong and well established allowing the programme to evolve and dive deeper into providing equitable health outcomes.

Marlborough Primary Health
PHOMarlborough Primary Health contracts health professionals and organisations in the community, such as General Practices, to deliver quality primary health care services to an enrolled population of approximately 46,600 people. There are currently eleven General Practices operating in the Marlborough district. Marlborough PHO and Nelson Marlborough Health are working in partnership to introduce and deliver the Health Care Home programme to the Marlborough district. Our focus is on improving access and equity, strengthening coordinated care for those with complex needs, and strengthening GP sustainability through implementation of the Health Care Home programme.

Nelson Bays Primary Health
PHONelson Bays Primary Health (NBPH) operates as a Charitable Trust with 21 general practices providing primary health care across the region with a population of 100,000. NBPH is working in alliance with Marlborough PHO and Nelson Marlborough Health to introduce the local Health Care Home programme to the Top of the South region for improved primary care access and options. Our vision is "Healthy people; Healthy workforce; Healthy community". Kia piki te ora ngā tāngata katoa.

Ngati Porou Oranga
PHOKaupapa MāoriNgāti Porou Hauora (NPH) is a Kaupapa Māori Primary Health organisation committed to delivering comprehensive healthcare services to the Ngāti Porou iwi and surrounding communities on the East Coast of New Zealand. Ngati Porou are passionate about service improvement and see Healthcare Home as a model of care that can improve service access and quality for their people.

Pegasus Health
PHOPegasus Health (Charitable) Ltd is a primary care network and PHO providing services and support to general practices and community based health providers within Canterbury to deliver quality healthcare to over 400,000 enrolled patients. Formed in 1992, we are committed to improving the health outcomes of the people in our community through a combination of service delivery, collaboration and partnerships.

THINK Hauora
PHOTHINK Hauora is a not-for-profit charitable trust, that provides primary healthcare services to the MidCentral DHB population. Primary healthcare services are managed and delivered in the community through the PHO. THINK Hauora has recently joined the Health Care Home collaborative and is working to deliver better access option for 60,000 people in the Horowhenua, Tararua and Manawatu areas.

Te Awakairangi Health Network
PHOHutt Valley District Health Board, Te Awakairangi Health Network and Cosine PHO work as an alliance and have collaboratively developed and funded the Hutt Valley Health Care Home programme in the district. In addition to the National Health Care Home model of care requirements, the Hutt Valley programme also focuses strongly on equity, consumer co-design and community integration. The Hutt Valley District currently has 15 active Health Care Homes, with approximately 80% of the local enrolled population with one of these general practices.

Te Puna Hauora Matua o Hauraki
PHOKaupapa MāoriHauraki Primary Health Organisation (PHO) is dedicated to providing high-quality primary healthcare services to their practice network. As a Kaupapa Māori PHO they are focused on improving health outcomes for all whānau within the communities they serve.

WellSouth Primary Health
PHOWellSouth Primary Health is a Primary Health Organisation serving the Otago/Southland region.

West Coast Health
PHOWest Coast Health's Community Wellbeing Services provide holistic, community-focused healthcare tailored to the unique needs of the West Coast region. With a strong emphasis on whānau-centered care, equity, and local collaboration, their services support mental, physical, and social wellbeing. By working alongside individuals and communities, they strive to ensure accessible, innovative, and sustainable healthcare that empowers people to thrive.
Western Bay of Plenty PHO
PHOWestern Bay of Plenty PHO provides primary health care services to 26 general practices across the Western Bay of Plenty sub-region, covering an enrolled population of over 165,000 patients, and a further 3 practices within Whakatane covering an enrolled population of almost 21,000.

Whanganui Regional Health Network
PHOWhanganui Regional Health Network (WRHN), formerly known as Whanganui Regional Primary Health Organisation, was established in July 2003 and is registered as a charitable trust, dedicated to improving health outcomes for communities across the Whanganui region. Through strong partnerships with general practices, iwi, and community organizations, they provide accessible, equitable, and whānau-centered primary healthcare services. Focused on innovation and collaboration, Whanganui Regional PHO supports the wellbeing of its communities by delivering high-quality care and addressing the unique needs of the region.

Pacific Health Services Hutt Valley
Community OrganisationPacific Health Services Hutt Valley is an independent health service working closely with our Pasefika community of 12,000 people in the Hutt Valley north of Wellington. We provide community engagement, development, support, health services and social services. Our focus is A Vibrant Healthy Pasefika community in the Hutt Valley. We have delivered value to Pasefika people since 1999.

Stroke Aotearoa NZ
Community OrganisationStroke Aotearoa New Zealand is the national, not‑for‑profit charity leading Aotearoa's fight against stroke. Established in 1979 from a partnership of volunteers and rehab professionals, it has since grown into the primary voice for stroke prevention, awareness, and support across the country.
